No, it doesn't hurt. A ball stretcher feels really good and it pushes the testicles away from the body. You may have done this before but not known it. I did it for years and didn't know that's what I was doing.

My experience with steel cock rings and O-ring types is that there is little difference between the two, once they're in place. Although the rubber rings are obviously more stretchy, this comes into play primarily when you're putting one on. After it's positioned at the base of your cock and down behind your scrotum, it's not being stretched, and therefore holds to its nominal diameter. In this way, a 2" steel ring and a 2" rubber O-ring perform nearly identically.

If your hard-on is strong enough to stretch the stout rubber of an O-ring to any degree, I'd aver that it's too tight to wear for any length of time and potentially dangerous, due to severe restriction of blood circulation.

I have two rings that are both worn the same way -- a 2" stainless steel ring, and a similar-sized silicone ring with an attached butt plug that's shaped like a giant thumb. With the type of ring I described, this wouldn't work. I've read that there are some pumps that are designed to be used with silicone/rubber cock rings. The ring is stretched around the pump. It's easier to put the ring on before you're erect, but I've managed to do it after too. As for the steel cock cage, I can't put that on if I'm erect. It is designed to go on a flacid cock, or semi-erect at the most.

It's also important to choose the correct diameter of the ring. If it's too small, you'll never get it around your junk and if you force it too much, you could cause yourself some serious injury. If it's too big, well, just don't bother because it won't have any effect.

So, how do you measure the circumference of your scrotum or your penis? The easiest way is to take a piece of string, wrap it around the base of your scrotum or cock. Pull it snug, and mark it. I was thinking of a simple silicon ring that just goes around the shaft (one came free with my penis pump). I was just a little afraid to use the ring without advice.
